I have run into a case with some physicians that have asked me to provide the study showing that the vial septum could be contaminated with microorganisms -- thus the need to swab the vial with alcohol.  Has anyone done or seen this?

The cover on a vial is a dust cover only.  That is an amazing question coming from a physician.  I would point you to the CDC website and all of their information on safe injection practices.  You can also give them a copy of the document from APIC!  I also included a CDC document (page 17) that provides evidence-based science on disinfecting vial septa.
