We are currently preparing phenol (various concentrations - for nerve block).  Does this need to be compounded by a pharmacy classified as a high risk pharmacy or is it acceptable for a medium risk facility to compound this.

R. Fall,

 

If the source ingredient (phenol) is not sterile, then it is a high-risk level CSP and needs to handled and compounded as a high-risk level CSP in an appropriate facility and QA/QC procedures according to the current USP Chapter <797>.
