We use a 1:256 dilution Quat ammonium detergent/disinfectant product called VIrex II (equiv to Lysol IC) to clean our ISO 5 surfaces daily followed with sterile water rinse and sterile IPA.  Environmental services prepares the solution from concentrate via a diluting dispenser using tap water.  My question is, should this product be diluted in sterile water for use on ISO 5 surfaces?  Thanks for all replies and input!

Doug,

Thanks for the post and I applaud your thinking.  I recommend to my clients that they use sterile water when preparing their detergent solution that is used to clean ISO Class 5 environments.  Although the Virex should kill any water-borne organisms, potable water is dirty and starting with sterile water ensures that the cleanliness of the most critical environment can be maintained and not inadvertently contaminated from our water.

Does anyone know if a product called Accel TB is equivalent to a Lysol IC? It states that Accel TB says it's a "ready to use disinfectant cleaner". I would like to find out if we can use this in place of mixing up Lysol IC daily. Any input would be appreciated. Lysol IC says it is a quat disinfectant cleaner and Accel TB is an accelarated hydrogen peroxide disinfectant cleaner.

Thanks

Patti,

Not sure how you are using the term "equivalent"?  They are both EPA approved disinfectant with similar cidal cleaning properties but Accel TB has a much better health and safety profile.  Lysol IC is good stuff but very corrosive and can pose an exposure risk to users.  Accel TB leaves no residue after cleaning and have been reported to be effective as a sporicidal agent.  It is more expensive but good stuff.  I would consult with your hospital's infection preventionist because they will be familar with this product and can give you additional support.

I was going by the recommendation that we should add a germicidal detergent. When I saw the Accel TB which came as a sample in a new glove box, I noted that it said cleaner and it did not need to be mixed up. We will use up the Lysol IC that we ordered but it would make it easier if we did not have to make our own dilutions up daily. I was wondering if it was an equivalent product in terms of being a germicidal detergent.

Thanks,

Patti

This is the posting I am referring to:

Patti,

A germicidal detergent should be used daily for your ISO Class 5 areas and work surfaces immediately around your ISO Class 5 areas.  The use of DECON-SPORE 200 Plus is good as disinfectant but it isn't a detergent which is needed to get dirt and grime off of critical surfaces.  Once of those washed surfaces are dried, you want to wipe them all down with sterile IPA to remove any detergent residue.

If you have spores, you may want to use a dilute bleach solution to disinfect the affect area. Bleach is a very effective sporicidal agent. Phenolic based cleaning agents are environmentally caustic and not recommended. You should look at using a quaternary ammonium disinfectant like Lysol IC, used daily with a dilute bleach solution once a week. Bleach will pit stainless steel and should be wiped off with sterile WFI followed by sterile 70% isopropyl alcohol.
